An Important Message about Commonwealth Care 1099 HC Information

January 16, 2013

Massachusetts law requires adult residents 18 years and older to have health insurance if they can afford it.

During the month of January, Commonwealth Care and former Commonwealth Care Bridge members with incomes above 150% of the federal Poverty Level (FPL) at any time during the calendar year, will receive a FORM MA 1099 HC from the Health Connector.

This form shows their health insurance coverage months and information about how to complete the Schedule HC.  Members should save this document; they will need it for income tax preparation.

Members should visit the Health Connector website at MAhealthconnector.org  to review 1099 HC information and view the step-by-step tutorial on how to review Commonwealth Care health insurance information.

They can also go to www.mass.gov/dor for more information.
